I acknowledge as Parent/Guardian that participation in baseball tryouts involves inherent risks and physical
activity. I voluntarily assume all risks associated with my child’s participation and agree to
release and hold harmless Fortitude Elite Athletics / Fortitude Baseball, its directors,
officers, coaches, staff, and volunteers from any liability, to the fullest extent permitted by
Florida law, except in cases of gross negligence or willful misconduct.
I, the undersigned parent or legal guardian of the participant listed below, hereby grant Fortitude Elite Athletics, Inc. permission to photograph, record, and capture my child’s likeness, image, voice, and performance during
tryouts, practices, games, training sessions, and organizational events.
I understand that such media may be used for organizational websites, mobile applications, social media
platforms, promotional and marketing materials, recruitment efforts, fundraising initiatives, and educational
purposes.
I acknowledge that no compensation will be provided for the use of such media and that Fortitude Elite Athletics
retains ownership of all media content. I further release and hold harmless Fortitude Elite Athletics, Inc., its
officers, directors, employees, volunteers, and agents from any claims arising from the use of such media,
including claims related to privacy or likeness.
This authorization remains in effect unless revoked in writing and does not apply retroactively to media already
